The Thyroid: Setting the Body's Pace

A butterfly shaped gland in your neck, small enough to sit in the palm of your hand, quietly sets the speed at which almost every cell in your body runs. Too fast or too slow, and the effects show up nearly everywhere at once.
The thyroid is a small, butterfly shaped gland sitting at the front of the neck, just below the voice box. Its job is deceptively simple to describe: it releases hormones that set the pace at which cells throughout the body use energy. Thyroid hormone as a dial, not a switch Thyroid hormone does not turn cells on or off. It adjusts how fast their everyday chemistry runs, rather like a dial controlling the speed of a motor. More thyroid hormone speeds this pace up. Less thyroid hormone slows it down. Because nearly every cell in the body responds to thyroid hormone in some way, changing this single dial produces effects across the whole body at once, rather than in one isolated organ. When the dial is turned up: an overactive thyroid When the thyroid releases too much hormone, the body's chemistry runs faster than it should. This can produce a fast heart rate, weight loss despite a normal or increased appetite, feeling too warm, sweating, tremor, anxiety, and difficulty sleeping. Each of these on its own could have many causes, but together they form a recognisable pattern pointing toward an overactive thyroid. When the dial is turned down: an underactive thyroid When the thyroid releases too little hormone, the body's chemistry slows down. This can produce tiredness, weight gain, feeling persistently cold, dry skin, constipation, and slowed thinking. This pattern is, in many ways, the mirror image of an overactive thyroid, which makes sense once you see both as the same dial turned in opposite directions. Why the thyroid needs iodine The thyroid gland requires iodine, a mineral obtained from the diet, to actually build its hormone. Without enough iodine, the thyroid cannot produce sufficient hormone even if every other part of the system is working normally. This is why iodine deficiency, though now uncommon in many countries because of iodised salt, remains a leading cause of underactive thyroid function worldwide. A gland controlled from elsewhere The thyroid does not decide on its own how much hormone to release. It is instructed by a signal from the pituitary gland, covered in more detail in the next lesson. This means a thyroid problem can originate in the thyroid itself, or in the signal instructing it, which is exactly why doctors check more than one hormone level when investigating thyroid symptoms. What a doctor checks A doctor investigating possible thyroid problems is essentially trying to work out which way the dial has moved, and why. The pattern of symptoms. A doctor asks specifically about heart rate, weight change, temperature tolerance, bowel habit, and energy levels, since the direction of change across these features usually points clearly toward an overactive or underactive thyroid. Physical examination. The doctor examines the neck for any visible or felt swelling of the thyroid gland, checks heart rate and rhythm, and looks for tremor, skin changes, or slowed reflexes. Blood tests. Thyroid function is usually assessed with more than one hormone level at once, since the pattern between them helps show whether the problem sits in the thyroid itself or in the signal controlling it. When this system is affected Both an overactive and an underactive thyroid are common, treatable conditions once correctly identified, but their symptoms can easily be mistaken for unrelated causes such as stress or normal ageing.
