The Lymphatic System: Where Immune Cells Live and Travel

When a doctor feels swollen, tender lumps in your neck during a throat infection, they are feeling your immune system at work, not a separate problem. Those lumps are lymph nodes, part of a whole network dedicated to producing, storing and deploying immune cells throughout the body.
Immune cells do not simply appear wherever they are needed. They are produced, matured and organised by a dedicated network of organs and vessels called the lymphatic system. This network works alongside the bloodstream but is structurally separate from it. Bone marrow: where immune cells are made Most immune cells, including the lymphocytes central to adaptive immunity, are produced in bone marrow, the soft tissue inside certain bones. Bone marrow constantly generates new immune cells throughout life, replacing cells that have completed their lifespan or been used up fighting infection. The thymus: where some lymphocytes learn to recognise self from foreign One type of lymphocyte, the T cell, leaves bone marrow while still immature and travels to a small organ called the thymus, located in the chest. In the thymus, T cells undergo a process that tests whether they react appropriately to the body's own tissues. T cells that react too strongly against the body's own cells are eliminated before they ever leave the thymus. This is a key step in preventing the immune system from attacking the body it is meant to protect. Lymph nodes: checkpoints where immune cells meet germs Lymph nodes are small, bean shaped structures scattered throughout the body, connected by a network of vessels that drain fluid from surrounding tissues. As this fluid passes through a lymph node, immune cells stationed there examine it for signs of infection or foreign material. This is why lymph nodes near an infected area often become swollen and tender. They are actively filtering fluid and mounting a local immune response. The spleen: filtering the blood itself While lymph nodes filter fluid from tissues, the spleen performs a similar filtering role directly on blood. It removes damaged or aged red blood cells. Its immune cells also monitor blood for signs of infection, making it an important site for mounting an immune response against threats that have entered the bloodstream directly. Why this network matters beyond individual organs Lymph nodes and the spleen bring immune cells into close contact with fluid or blood that may carry germs. This greatly increases the chance that the right lymphocyte encounters the right target quickly. Without this organised network, immune cells would rely on random chance to encounter a specific threat, which would make an effective, timely response far less likely. What a doctor checks A doctor examining the lymphatic system is often looking for evidence of where the body is actively fighting an infection or, less commonly, other underlying disease. Palpating lymph nodes. A doctor feels for swollen, tender lymph nodes in areas such as the neck, armpit and groin, noting their size, texture and location. Location pattern. Because lymph nodes drain specific regions of the body, which nodes are swollen can point toward where an infection or other problem is located. Imaging and blood tests. When swelling is persistent or unexplained, imaging or blood tests can help distinguish a straightforward infection from other causes of lymph node enlargement. When this system is affected Lymph nodes can remain swollen for an unusually long time, or swell without an obvious infection nearby. This can be a sign that something other than a simple, resolving infection is affecting the immune system. This is explored further in the lesson "Infection: When Something Else Gets Inside".
