The Brain's Main Regions and What They Do

The brain is not one uniform blob doing everything at once. Different regions specialise in different jobs. Knowing roughly which region does what explains why damage to one small area can affect one specific ability while leaving everything else untouched.
The brain is often drawn as a single smooth shape. In reality, it is made up of distinct regions, each contributing a different kind of work to the whole. The cerebrum: the large, wrinkled outer brain The cerebrum is the largest part of the brain, the wrinkled outer structure most people picture when they think of a brain. It is divided into a left and right half. It is further divided into regions called lobes, each associated with different functions. The frontal region toward the front is heavily involved in planning, decision making, and movement. The region toward the back is heavily involved in processing what you see. Regions at the sides are heavily involved in hearing and language. Regions toward the top and back help process touch and spatial awareness. These regions work together constantly, so few real tasks belong to just one of them alone. The cerebellum: fine tuning movement and balance Tucked beneath the back of the cerebrum sits the cerebellum, a smaller, densely folded structure. Its main job is coordinating smooth, accurate movement and maintaining balance. It does not initiate movement itself. Instead, it constantly fine tunes movements the cerebrum has already set in motion. This is why damage here tends to produce clumsy, poorly coordinated movement rather than weakness itself. The brainstem: running the automatic essentials Below the cerebrum and cerebellum, the brainstem connects the brain to the spinal cord. It is a relatively small structure. It controls functions essential to staying alive: the automatic control of breathing rate covered in a previous lesson, as well as heart rate, blood pressure, and consciousness itself. Because so many essential automatic functions run through this small area, damage here is disproportionately serious compared with damage of a similar size elsewhere in the brain. Why location changes what a symptom means Because different regions specialise in different jobs, the same general problem, such as reduced blood flow, produces very different symptoms depending on exactly where it occurs. A problem in a frontal region might affect movement or decision making. A problem toward the back might affect vision. A problem in the cerebellum might affect coordination and balance without any weakness. A problem in the brainstem might affect consciousness or basic survival functions directly. This is why doctors pay close attention to precisely which abilities are affected, not just that something is affected. What a doctor checks A doctor examining someone with a suspected brain problem is, in effect, testing different regions in turn to work out where the problem most likely sits. Movement and coordination. Testing strength points toward the cerebrum's movement regions or the pathways leading from them. Testing coordination, such as touching the nose then the doctor's finger, points more toward the cerebellum. Vision, speech and sensation. Testing vision, language, and sensation in different parts of the body helps localise a problem to specific cerebral regions responsible for each. Consciousness and vital functions. Checking level of consciousness, breathing pattern, and basic reflexes helps assess whether the brainstem itself is affected. This is treated with particular urgency, given how much depends on this small structure. When this system is affected A stroke affecting one specific brain region typically produces a specific, recognisable pattern of symptoms rather than a vague, general decline. This is part of why sudden, localised symptoms such as one sided weakness or sudden vision loss are treated as a medical emergency. This is explored further in the lesson "Weakness or Numbness".
