How New Information Changes a Doctor's Thinking

New information can support, weaken or leave a working diagnosis largely unchanged. This lesson explains how clinicians update probability without treating one symptom, test or follow up result as a final verdict.
Updating, not guessing A working diagnosis is a provisional explanation, or set of explanations, that guides the next step. It remains open to revision until enough evidence supports safe decisions. Relevant new information can support, weaken, add, remove or reorder possibilities. It may also leave the current assessment largely unchanged. Information includes what the person describes, the examination, test results, treatment response and changes over time. Clinicians do not need absolute certainty before acting. They need enough certainty to choose a proportionate and safe next step. A diagnosis considered confirmed can still be reconsidered if later evidence conflicts with it. How can the same test result mean different things in different people? The answer begins with probability before testing. Probability before and after a test Pre test probability is the estimated likelihood of a condition before the new result is known. It comes from symptoms, signs, background risk and how common the condition is in that setting. Post test probability is the updated likelihood after the result. It depends on the starting probability and how well that test performs in that situation. The same result can therefore have different meanings in different people. The result is being combined with different starting information. This does not make testing subjective. It means that a result is interpreted as one part of an evidence based clinical assessment. Positive and negative do not always mean present and absent Some tests give positive or negative results. Others give measurements that are interpreted against an expected range or a decision cut off. A false negative occurs when a test misses a condition that is present. A false positive occurs when a test suggests a condition that is absent. Timing, sample quality, medicines, diet, disease stage and the chosen cut off can affect interpretation. The importance of each factor depends on the particular test. Some tests can establish a diagnosis in a defined setting. Many only raise or lower probability, and some require confirmation with another assessment or test. A result should answer a specific clinical question. It should not be used as a general verdict on every possible cause of a symptom. Normal and abnormal results need context A normal result may be reassuring, but it does not automatically explain persistent symptoms. An abnormal result may be important without identifying its cause. Clinicians ask what the test measures, what it can miss and whether the result fits the complete picture. If a result conflicts with a strong clinical pattern, options may include checking timing, repeating testing, choosing another test or seeking specialist advice. These steps depend on the condition and the clinical pathway. You should not independently ignore a result or decide that a condition is present. Time and response provide new information Symptoms improving as expected may support a working diagnosis, but improvement rarely confirms it alone. Lack of improvement, recurrence or a new symptom can weaken the current explanation or reveal another problem. Non response may reflect an incorrect diagnosis, an evolving illness, more than one cause, or treatment that was ineffective or not used as intended. Response to treatment can also be non specific. It should not be treated as a diagnostic test unless evidence supports that use. Safety netting makes follow up information usable. It explains the expected course, concerning changes, review timing and how to seek help. When a plan changes A revised plan can show appropriate updating. It does not automatically prove that the earlier assessment was correct, careless or unavoidable. An earlier decision should be judged against the information, urgency and reasonable options available at that time. Good care also requires acting on new information, arranging follow up and responding when the expected course does not occur. Ignoring conflicting evidence, delaying required action or failing to arrange review can create diagnostic harm. Choosing and timing tests Tests are most useful when they answer a defined question and could change management. Testing too broadly can produce unexpected unrelated findings, false positives and further procedures that do not help. Timing also matters because some tests become informative only under certain conditions. However, urgent or time sensitive testing must not be delayed simply to gather more context. Deliberate monitoring is appropriate only after assessment, with a review plan and safety netting. This is different from postponing a test that a clinical pathway requires now. A practical example: coeliac disease testing Coeliac disease shows why test conditions matter. Serology usually includes immunoglobulin A, called IgA, and tissue transglutaminase antibody, called tTG. NICE states that testing is accurate only while eating a gluten containing diet. IgA deficiency can require different antibody testing. If serology is negative but coeliac disease remains clinically suspected, NICE advises gastrointestinal specialist assessment. Starting a gluten free diet before assessment can make confirmation harder. Discuss testing and diet changes with a healthcare professional. Questions you can ask You can ask what the test measures, how much it changed the working diagnosis and whether timing affected interpretation. You can also ask what happens next, when review is planned and which changes should prompt earlier help. These questions support shared understanding. They do not provide a method for interpreting your own results without clinical context. Seek urgent care for severe or rapidly worsening symptoms, regardless of a previously reassuring result.
Relevant information updates probability rather than replacing the whole clinical picture. A test result can raise or lower likelihood, but its meaning depends on performance, timing and pre test probability. A changed plan can be appropriate updating without proving that the earlier plan was safe or unsafe.

- Coeliac disease
- An immune condition in which gluten damages the lining of the small intestine. Symptoms and blood tests alone may not confirm it.
- Serology
- Blood testing that looks for immune-system markers linked to a condition. Its meaning depends on the particular test and clinical context.
- Immunoglobulin A (IgA)
- A protein made by the immune system and measured during coeliac testing. Low IgA can make standard IgA-based tests less reliable.
- Tissue transglutaminase antibody (tTG)
- An immune protein commonly measured when investigating coeliac disease. The result must be interpreted with diet, total IgA and the wider assessment.
